Registering with us takes three steps. You contact the clinic and tell us you are looking for a family doctor; we confirm whether we have capacity and book you an intake appointment; you attend that appointment with your health card, medication list and your previous clinic’s details, and we handle the records transfer from there. If we are temporarily full, we add you to the new patient list and contact you when a spot opens. You can also be seen in our walk-in clinic in the meantime, so you are never left without care while you wait.
Before you start: what you will need
| Bring | Why |
|---|---|
| Valid Ontario health card | Required for OHIP-covered care |
| Your previous clinic’s name, address and phone number | So we can request your records |
| Your previous doctor’s full name | Records are filed under it |
| Any former or maiden name | Your old records may be under it |
| A list of current medications and doses | Including over-the-counter and supplements |
| Allergies | Medications, food, environmental |
| Major conditions, surgeries and admissions, with approximate dates | Builds your problem list |
| Specialists you currently see | So we can continue those relationships |
| Immunisation record, if you have one | Hardest thing to reconstruct later |
| Family history of significant illness | Changes when screening starts |
| Your pharmacy’s name and location | For prescriptions and refills |
| Dates of your last screening tests | Pap, mammogram, colon screening, bloodwork |
Do not delay registering because you are missing some of this. Come with what you have; we will fill the rest in over time.
Step 1 — Contact the clinic
Call us, visit reception, or use the new patient enquiry form on miltonhealthcentre.com.
Tell us:
- You are looking for a family doctor
- How many people in your household need one, and their ages
- Whether anyone has a condition needing ongoing management
- Whether you currently have a family doctor, and where
If we have capacity, we will book your intake appointment.
If we are temporarily full, we will take your details for the new patient list and contact you when a spot opens. Ask us roughly how long that usually is, and keep looking elsewhere in the meantime — being on more than one list is sensible, and you can tell us if you are accepted somewhere else.
Step 2 — Your intake appointment
This first visit is longer than a normal appointment because it builds your chart from nothing.
What happens:
- Reception registers your health card and contact details
- You complete a patient intake form — history, medications, allergies, family history
- You sign the records release form so we can request your previous file
- If applicable, you complete patient enrolment paperwork, which we will explain before you sign
- The doctor reviews your history with you and examines you as needed
- We identify what is overdue — screening tests, immunisations, bloodwork, chronic disease checks
- You leave with a plan: requisitions, prescriptions, referrals, and a follow-up date
Come with your questions written down. This is the appointment to ask about appointment waits, after-hours care, how results are communicated, and anything you want to know about how the practice runs.
Step 3 — Your records transfer
We send the request to your previous clinic using the release form you signed. Most transfers arrive within two to four weeks. There is often a cost-recovery fee charged by the sending clinic, and they will tell you the amount.
You do not need to contact your old doctor yourself, and you do not need to explain why you are leaving.
If your previous practice has closed or your doctor has retired, tell us — records are still retained somewhere, and we can help track down who holds them.
While you wait for a spot
If we cannot register you immediately, you still have options:
- Use our walk-in clinic for anything that comes up — same clinic, same building
- Call Health811 at 811 any hour for nurse advice on where to go
- Register with Health Care Connect, Ontario’s provincial matching service, which can refer you to practices accepting patients
- Keep your medication list current so any doctor you see has what they need
- Go to the emergency department for anything urgent — see the red flags in the articles linked below
Do not skip screening while you wait. If you are due for a mammogram, colon cancer screening or a Pap test, our walk-in doctors can order these.
What we offer new family practice patients
- Family medicine for adults and children
- Walk-in care in the same clinic, so urgent problems do not need another provider
- Chronic disease management — blood pressure, diabetes, asthma, cholesterol, thyroid
- Preventive care and cancer screening
- Immunisations on Ontario’s publicly funded schedule
- Minor procedures in-office
- Mental health assessment, treatment and referral
- Specialist referrals, with follow-up tracked by the clinic
- Prenatal and postnatal care
- Travel health and our travel vaccine clinic, including yellow fever
- Virtual and phone appointments for appropriate problems

Registering your family
Tell us at the first contact who else in your household needs a doctor. Where we have capacity we keep families together, and we can often book family appointments back-to-back. If we can only take some members now, we will add the rest to the list.
FAQ
Is there a fee to register or to join the waiting list?
No. Charging a fee to be accepted as a patient or to join a list is not permitted in Ontario, and we do not do it.
Do I need an OHIP card to register?
You need a valid Ontario health card for OHIP-covered care. If you are new to Ontario, newly arrived, or waiting for coverage to begin, contact us anyway — we will explain your options and what a visit costs in the meantime.
Can I be seen before my records arrive?
Yes. Your intake appointment does not wait on the transfer. Bring your medication list and we will work from that.
Will I always see the same doctor?
We aim for continuity with your own physician for family practice appointments. For urgent same-day problems you may see another doctor in the clinic, who has access to your chart.
What if I already have a family doctor but want to switch?
You can switch without permission, a reason, or a conversation with your current doctor. Contact us the same way as anyone else; we handle the records request.
How long is the wait for an appointment once I am registered?
Ask us at registration for the current answer — it changes. We also hold same-day slots each day for urgent problems.
Do you see children and babies?
Yes, including newborn visits, growth and development checks, and the publicly funded immunisation schedule.
What if I need a specialist I already see?
Tell us at intake. We will continue that relationship and request their reports so your chart is complete.
What should I do if my health gets worse while I am on the waiting list?
Use our walk-in clinic, or go to the emergency department for anything urgent. Do not wait for a registration spot to address a new or worsening problem.
